NTC employees should complete this form to request alternate work arrangements due to issues related to COVID-19 for the 2021 Spring Semester. NTC Leadership is aware that everyone's personal situation is fluid and can change at any moment.  At the same time, the College needs to continue working toward the common goal of providing a fantastic undergraduate experience to our students.  This includes holding in person 1-on-1 meetings with students as the default option.  Approvals for all requests will come from the Dean's office in coordination with the Senior Associate Dean of your area.  Your supervisor will also be copied on the request and approval.
 
Special Note: 
If you’re requesting an accommodation due to your own health status, you MUST go through the University’s ADA request process.  Staff should contact the Office for Campus Accessibility at ADAaccess@tulane.edu to request ADA Accommodations. Communications regarding potential ADA accommodations are confidential and will not be shared outside of the Office for Campus Accessibility without the employee’s consent.
