Sunday October 13, 10 a.m. - 4 p.m.

Applications may not be accepted/considered after June 30, as space is limited. 
 
General Information
 
* No payment is due with the application. Booth fees will be due upon notification of acceptance.
* A committee will review applications and select food vendors to participate in the event.  The number of food vendors selected in any given category may be limited to minimize redundancy. 
* Accepted food vendors will be notified via email of their acceptance no later than July 19. Payment instructions will be included with the acceptance notification.
* IMPORTANT: Booth fees for ACCEPTED food vendors will be due no later than August 16. Booth fees not paid by that date may forfeit the booth space and/or incur a $25 late fee. No booth fee refunds will be granted after September 1, 2019.
* Corner weights are mandatory.  The weight requirement is 50 lbs per corner.
* Food vendors must provide their own means for electronic fund transfers (e.g. Squareup) and cash to make change.
* Completion/submission of this form incorporates applicant acceptance of our  Terms and Conditions as stated below.
* This is a rain or shine event.

(PLEASE NOTE - these prices are for applications received on or prior to June 30, 2019. Booth price after that date, in the event late applications will be accepted, will incur a $25 late fee in addition to the booth fee, if accepted.

1) Your Food Handlers Permit, and
2) Temporary Food License (issued by Southwest Utah Public Health)

Terms and Conditions

To be eligible to participate in Art in Kayenta 2019 you must agree to:

  • Participate in all three days of Art in Kayenta (AIK)

  • Set-up on Thursday, October 10th by 4:00 pm.

  • Accept the vendor space assigned (outdoors in a 10’ x 10’ or 10’ x 20’ space)

  • Post a menu with your business name

  • Bring and use a WHITE canopy with corner weights of not less than 50 pounds each unless another set-up is approved by KAF in advance

  • Use a code-compliant food surface that provides a smooth, cleanable, non-absorbent surface, IF you will be using a BBQ or cooking with grease

  • Provide your own heavy duty extension cords

  • Bring all equipment and supplies you will need for the event including ice, trash bags, money for making change and equipment for accepting credit/debit cards

  • Acquire and post 1) a Food Handlers Permit and 2) a Temporary Food License as issued by Southwest Utah Public Health. Contact the Environmental Health Division at 435-986-2580 or visit their website www.swuhealth.org

  •  Report and pay appropriate State sales tax.

  • Accept $5.00 vouchers issued by Kayenta Arts Foundation toward purchases and submit such vouchers to KAF at the end of the event for check reimbursement by mail, folloiwng the event. 

  • Act in a professional manner, bringing any problems to the attention of the KAF representative whose name will be provided to you.

  • Remove your equipment and leave your assigned space clear of debris no later than 7:00 p.m. Sunday, October 14, unless other departure time is approved by KAF in advance.

By signing and submitting this Application and Agreement, you certify that you have read and agree to comply with the above, and you understand that KAF reserves the right to end your participation for failure to do so or for any good cause. Selection of participating vendors is at KAF’s sole discretion. Participation in Art in Kayenta is at your own risk; you accept the site as is, you are responsible for the security of your materials and equipment and safety of your staff. By your signature below, you forever release, discharge, and hold harmless Kayenta Arts Foundation, Coyote Gulch Owners Association, Kayenta Development, and sponsors of Art in Kayenta, their directors, officers, employees, agents, assigns, and volunteers from any and all responsibility, personal liability, claims, loss, or damage, of any kind arising out of or in conjunction with your participation Art in Kayenta.

Vendors will be selected at KAF’s sole discretion. KAF will notify each vendor of whether the vendor has been accepted by Juy 19, 2019.
