This form must be completed for each child enrolled in any drop-off program at Peekadoodle.

By signing below, I acknowledge that I am aware of the risk of injury or illness inherent in any activity or program designed for young children. As the parent or legal guardian of the child above, I understand that Peekadoodle (PKC) is permitting my child to participate in Peekadoodle upon the express understanding that I hereby waive and release PKC, its employees, agents, officers, and directors from any and all claims arising from my family's activities at PKC, and indemnify and hold PKC harmless from and against any and all liabilities, expenses or judgments, including attorneys fees and court costs resulting therefrom.

I acknowledge that PKC does not have licensed medical staff available onsite and is not obligated to administer any medical care or medications. In the event of an emergency related to my child, I understand PKC will use reasonable efforts to contact me, the named emergency contacts or named family physician. If PKC is unable to reach me or my named contacts, I give permission to PKC to seek whatever medical care the child needs as determined by a PKC employee in her or his sole discretion and hereby release PKC from all liability resulting therefrom. I further agree to be responsible for any and all charges for the emergency medical care.

I hereby give permission for my child[ren]  to participate on walking field trips around the camp location. I authorize my child to take part in all normal activities conducted. I understand that numerous field trips within walking distance, will occur periodically during each camp week. I agree that while precautions will be taken to keep my child injury free, I understand that accidents happen and accept full responsibility for any costs related to any accident or injury. 

I understand that I will apply sunscreen in the morning before sending my child to camp. I authorize Peekadoodle staff to apply the following non-prescription sunscreen to my child, in the afternoon as needed. I understand that this product will only be applied according to labeled directions. Any deviations from labeled directions will require a treating physician's written authorization. This authorization is valid for 90 days.
