This checklist must be reviewed and completed prior to any work being commenced by Health & Life on the above-nominated entity. Completion of all areas of the checklist will assist us in preparing your financial statements and income tax return in both a timely and cost-effective manner.
Skipping questions may result in extra costs and a delay on the annual accounts as Health and Life will be required to prepare a Request for Further Information.

Please note estimated turnaround time of completed accounts is 8-12 weeks. This may vary depending on how complete and timely the information has been provided by you. This will keep your fees down. We may be able to provide a shorter time frame as long as this is not the first year of restructuring and our instructions are followed on a timely basis.
We will prepare a detailed Management Letter aimed at improving the sustainability and profitability of your practice. The recent amendments to the Tax Agents’ Regime have seen a greater responsibility on Tax Agents to ensure that all claims by clients can be fully substantiated. Accordingly, we may need to request more information than in earlier years. Unfortunately, this may result in higher preparation costs and delays if our advice is not being followed.

This form can be used to collect and submit data for all trusts, companies, partnerships and family trusts to us. This form is required to be submitted for each of the entity types that you use. This has been specified in the end of year checklist email sent to you. You will not be required to re-upload information, e.g. a signed Legal & Tax Structure. Once you have completed this form for one type of entity e.g. a Medical Partnership and hit submit, you will then be prompted to click the link again to complete your next entity e.g. a Service Trust. Once you have provided information for all entities on your Legal & Tax Structure requiring an End-of-Year Checklist, we will then start working on your file.
