A non-refundable deposit of $150.00USD on the DIVA (backline), $200.00USD on all BELLA (full coverage)$250.00USD on all VIXEN (Frontline) costumes is required on all registrations with the balance due no later than May 1, 2021. There is an additional $50 USD required on all feathered upgrades. This deposit is necessary to secure your costume and your participation in Fuzion Mas St. Lucia 2021. 50% of the cost of the costume must be made by January 31, 2021. Should you fail to comply with the payment arrangement we reserve the right to place your costume back into inventory.

 

(Please note that there is an additional processing fee on all online transactions which will be added to your final bill). Kindly note that once you have selected your costume, there will be absolutley no switching. Costumes will be defaulted as of May 15th, 2021. Which means, without full payment on May 15th 2021 your costume  will be placed back into inventory. NO EXCEPTIONS!

 

I understand that upon submiting this registration form, I am responsible for the payment of my selected costumes as laid out herein and I am fully understanding of the terms and conditions thereof.   I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH LIV Carnival, Fuzion Mas, and any of their agents and representatives, and all who are being represented, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.   I, the undersigned do herby consent and agree that Fuzion Mas, its employees or agents, have the right to take photographs, videotape, or digital recordings of me and to use these in anyand all media now or hereafter known. I release all rights to exhibit this work in print and electronic form publicly or privately and to market. I understand that there will be no financial or other remuneration for recording me. I represent that I am at least 18 years of age. If under the age of 18 years old you must bring a parent or guardian to sign a release form, before the costume is released. I particularly understand that ALL DEPOSITS ARE NON-REFUNDABLE. In the event that the Carnival is cancelled postponed or delayed through no fault of LIV Carnival, as deposits are used to acquire material to secure the product which we plan to deliver. However Liv Carnival will make every effort to refund deposits if the carnival is cancelled or postponed by reselling all costumes as requested by the costumer (we cannot however make any guarantees to same). I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
