COSTUMES WILL BE AVAILABALE FOR PICKUP ON OCTOBER 2nd in NY and OCTOBER 7th and 8th in MIAMI. LOCATION TO BE ANNOUNCED!!!

Kindly note that once you have selected your costume, there will be absolutley no switching. Costumes will be defaulted as of September 17th, 2021. Which means, without full payment on September 17th 2021 your costume will be placed back into inventory. NO EXCEPTIONS!

I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. I, the undersigned do herby consent and agree that Fuzion Mas, its employees or agents, have the right to take photographs, videotape, or digital recordings of me and to use these in anyand all media now or hereafter known. I release all rights to exhibit this work in print and electronic form publicly or privately and to market. I understand that there will be no financial or other remuneration for recording me. I represent that I am at least 18 years of age. If under the age of 18 years old you must bring a parent or guardian to sign a release form, before the costume is released.
