Wasatch Figure Skating Club – Learn to Skate (Weber County Sports Complex)

2021 Winter Exhibition 

Date: Saturday, December 18th, 2021, 7:30 p.m.

Cost: $30

Rehearsals: Wednesdays 5:00 - 5:30 November 17 - December 15

Signing up/Due Date: Complete the requested information below and submit on or before Friday November 12th.

Payment: Turn in your payment (Cash or Check - Checks made out to WFSC) to Coach Letitia Ahrensback during Learn to Skate (you can bring it with you the first night of rehearsals as long as you have registered online) or Venmo payment to her @Letitia-Ahrensback be sure to put WFSC Winter Exhibition with your skater’s name in the notes. Skater requirements: Skaters must be able to skate to the middle of the ice on their own, with the group (without a coach) and be able to stay with the group of skaters during the performance. Note: There will not be a coach on the ice with skaters during the performance.  

Rehearsals dates:           

Wednesday November 17 5:00 – 5:30  

Wednesday November 24 5:00 – 5:30  

Wednesday December 1 5:00 – 5:30    

Wednesday December 8 5:00 – 5:30  

Wednesday December 15 5:00 – 5:30     

Attendance is important for skaters to learn their program in the short rehearsal time that we have. Skaters may not miss more than one rehearsal, or they will not be able to perform. Please inform Coach Letitia if you will be missing. Skaters are expected to be on the ice ready to go at 5:00 so we can start our full 30 min rehearsal.  

Groups: During the Winter Exhibition all ages and levels will be together. (During our Spring Show Productions Skaters are split into levels).

Costumes: Skaters will be responsible for their own costumes. To keeps things simple costumes will be winter attire, cute sweaters, scarfs, hats, mittens etc. skating attire if you have it.

Mask Mandate: The Wasatch Figure Skating Club Winter Exhibition is a US Figure Skating sanctioned event. US Figure Skating requires all sanctioned events to follow a 100% mask mandate. All participants, coaches, chaperones, spectators, volunteers, etc are required to wear a mask at ALL times while inside The Weber County Sports Complex during the Winter Exhibition. Failure to comply with this rule will result in immediate dismissal from the facility. There will be NO exception to this rule while off ice.

(on ice) The one exception is that when a performer(s) is on the ice no mask or face covering will be required for that performer(s) while on the ice. Mask will be required before and after performance. Masks will be worn at all times while not on the ice. Skater may have the option to still wear a mask if they wish to do so.

PARTICIPANT AGREEMENT I do hereby give my permission for my child/skater (participant as named above) to participate in the WFSC 2021 Winter Exhibition. I have read through and agree to the terms of this agreement and have been given every opportunity to ask questions though either email or text. I understand that my failure to follow these guidelines may result in my child/skater not participating the WFSC 2021 Winter Exhibition.

PARENTAL CONSENT, INDEMIFICATION AND CODE OF CONDUCT AGREEMENT

I (Parent or Guardian) understand that figure skating can be a challenging and dangerous sport and certify that my child is in reasonable health and that I will not hold the Wasatch Figure Skating Club responsible for any injury that my child may incur while participating in, or volunteering at or otherwise engaged in figure skating.

I also understand that I will not hold liable any member of the Wasatch Figure Skating Club, including its Board of Directors, for any injury incurred while engaged in figure skating.

I will uphold the US Figure Skating Code of Conduct and be respectful to parents, coaches, rink staff and other skaters.

I understand that if I feel I need to make a claim against a member of the Wasatch Figure Skating Club for an incident that has occurred at a US Figure Skating Sanctioned event, including shows, test sessions, and competitions, the Weber County Sports Complex, or at any Wasatch Figure Skating Club sponsored activity, I need to write and sign a formal complaint addressed to the Executive Board Members so that the US Figure Skating Code of Conduct, USFSAA GR 1.02 may be enforced. I understand the difference between the Wasatch Figure Skating Club and Weber County Sports Complex sponsored activities but recognize that Wasatch Figure Skating Club members represent US Figure Skating at all times. I also understand that I am entitled to discuss the complaint with the presence of one or more Executive Board members in confidentiality, but I may not discuss the complaint with anyone who is not directly involved with the situation. I also understand that gossiping and telling untrue things about the Wasatch Figure Skating Club members will lead to my dismissal from and forfeit of fund paid to the Wasatch Figure Skating Club and I will be reported to the US Figure Skating as a member not in good standing, which jeopardizes my continued eligibility and participation at all levels of figure skating.

COVID-19 Policies and Procedures and agreement

Additional Rules and Restrictions:

  • Each skater will be limited to 1 family member only accompanying skaters in the lobby/backstage. All other family members/friends may spectate the show from the stands following social distancing guidelines.
  • Performers, volunteers, coaches, and spectators will be asked to stay home if they have or are experiencing any of the following symptoms: DO NOT COME TO THE RINK! Cough, Trouble breathing, Shortness of breath, Sore throat, Sudden change in taste or smell, Fever/Chills, Unexplained muscle aches or pains, OR if they have had known contact in the past 14 days with someone who has been diagnosed with COVID-19
  • Skaters must follow flow signage as well as social distancing guidelines when entering and exiting ice appropriately.

I have read the above rules and agree to follow in order to participate in the 2021 Winter Exhibition. I understand that failure to do so may result in my being asked to leave the program and/or performance.

COVID-19 Waiver and Release

COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal authorities and the State of Utah recommend social distancing to prevent the spread of COVID-19. Contracting COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participation in Wasatch Figure Skating Club (WFSC) programs could increase the risk of you or your child contracting COVID-19. WFSC and its Board/Coaches/Members undertake every effort to follow guidelines for a healthy environment; however as with any public event, we cannot guarantee that you will be 100% safe from airborne illnesses such as COVID-19 or colds and flu while participating in any WFSC programs.

I understand and appreciate the risks that are inherent in participating in WFSC programs, and hereby assume all risks of illness, including death, that I may sustain while participating in or as a result of participation in WFSC programs. I hereby assert that my participation is voluntary and that I knowingly assume all such risks.

Completing this online form is an agreement that the skater and parent have both read the entire application and will follow ice show guidelines. Both the skater and parent will do what they can to make this the best experience for everybody involved. Please fill out and return as soon as possible - Due by November 12th.
