PARTICIPATION WAIVERS

Your responses below indicate full acceptance and understanding of these waivers and all related Forefront Arts policies.
ENROLLMENT & WITHDRAWAL TERMS
I understand that Forefront Arts makes arrangements based on student enrollments and this posted withdrawal/refund policy that I am agreeing to before registering my child applies to ALL campers, including my family.

I understand that camp registrations may be cancelled in writing by using the "Summer 2024 Cancellation Request Form" linked in my confirmation email and on the Forefront Arts website, until May 1, 2024 with a full refund given, less the credit card fees and a $50 administrative fee per camp spot. I understand that no cancellations, refunds, or credits will be given after May 1, 2024.

I understand that camp registrations may be transferred to a different week by using the "Summer 2024 Transfer Request Form" linked in my confirmation email and on the Forefront Arts website, until 2 weeks (14 days) before the original session starts dates. If space is available a transfer to another camp will be processed for a $25 transfer fee per spot. Families will be responsible for the $25 transfer fee and any cost difference between the original and replacement camp week tuition. I understand that transfer requests submitted with less than 14 days before the original session dates will be considered on a case-by-case basis and are not guaranteed. I understand that transfer requests cannot be processed within 7 days of the original session dates.

I understand that Summer Stock registrations are non-cancellable, non-refundable due to the nature of the Summer Stock program. I understand that a Summer Stock registration cannot be transferred to another program. If my child is not able to use his purchased Summer Stock spot we have the option to sponsor another student to participate in my child's spot.

I understand that if I take advantage of the "SUPER SIX" promotion, all camp registrations associated with that promotion & refund are non-cancellable once the 6th camp week's tuition has been refunded to me. I understand that transfers, refunds, or adjustments of any kind will not be possible within 7 days of the camp start date, or for any camper who has come to the camp and then chooses to not participate.

I understand that not participating because of the instructor or other campers (including their race, sex, age, nationality, or religion) or because of the role received is NOT a valid reason for withdrawal, and no adjustments can be made. I understand that I have the option to sponsor another child to participate in my child's spot if my child is not able to participate and a camp transfer or camp cancellation is not possible. *

LIABILITY / EMERGENCY MEDICAL CARE / PHOTO RELEASE

Forefront Arts provides classes and training at the exclusive risk of the participants.  It is strongly recommended that students are covered by their own family insurance policy.  Forefront Arts  is not responsible for any damage or loss to personal property or injury suffered during any associated event or activity.  Your response below indicates full acceptance and understanding of this liability waiver and all Forefront Arts policies.

I understand and agree that in participating in any performing arts program or recreational activity, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any classes or activities held by Forefront Arts. I also exempt, release, and indemnify Forefront Arts, and their owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, illness, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by the afore mentioned parties. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold the afore mentioned parties liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

I understand that I am responsible for any damages caused by my child to the host location, Forefront Arts property, or the property of another student.

I give permission to Forefront Arts to authorize any medical attention needed to my child/student in the event of a medical emergency. I understand that I am financially responsible for any damages caused by my child to the host location, Forefront Arts property, or the belongings of another student.

I hereby authorize and consent that Forefront Arts shall have the absolute right to copyright, publish, or use any and all photographic portraits or pictures, videotapes and/or sound recordings, or any part thereof, they have taken or made of my child, or in which my child may be included in whole or in part.

HEALTH & WELLNESS /COVID / Illness Cancellation Policy:

I understand that all 2024 Summer camp registrations are non-cancellable and non-refundable after May 1, 2024. I understand that Forefront Arts staff will do everything in their power to accommodate a transfer request if my child cannot attend their original camp week due to covid diagnosis or other illness, but they cannot guarantee that a transfer will be possible. Students and Immediate Family Members are fully aware of the risks and hazards with respect to COVID-19, or any other viruses, inherent in their participation in Forefront Arts programs. Students and Immediate Family Members freely and voluntarily agree to assume the risk with respect to COVID-19, or any other viruses, including the risk of death, bodily injury or property damage, regardless of severity, that they may sustain as a result of their participation in Forefront Arts summer activities. Any students found to be exhibiting a temperature above 100.4 or presenting Covid-19 symptoms will be immediately sent home until they are able to return, following current state & CDC guidelines. No refunds or credits will be given for students sent home for exhibiting Covid-19 symptoms or other illnesses but every attempt will be made to include the camper when they are well enough to rejoin their camp week.

CODE OF CONDUCT & BEHAVIOR EXPECTATIONS

We want our drama camps to be a fun and safe experience for EVERYONE involved. Parents, please review these expectations with your camper before your camp week.

We want everyone to have a great time in our program, and we have set 3 basic rules for Forefront Arts programs to ensure that everyone is safe and able to learn and have fun:
1. Respect the teacher: "Listen while the teacher is talking, and do what they say". Students are expect to follow the directions of the Forefront Arts staff. Students are expected to participate in all scheduled activities and maintain a positive attitude.
2. Respect each other: "Keep your hands to yourself and only say kind things". Students are expected to treat everyone with respect: bullying, stealing, teasing, cut-downs and profanity will not be tolerated. No touching other students or staff is allowed. Students are expected to respect personal space and make adjustments if reminded they need to spread out.
3. Respect our space: "Don't mess with anything that is not yours". Students are expected to care for our host locations and not purposely deface the host locations, or purposefully harm or destroy any property, furniture, props, or costume pieces, or items belonging to other students.

PROCEDURES FOR DEALING WITH INAPPROPRIATE BEHAVIOR:

Our three code of conduct rules will be reviewed with all campers every morning during circle time.
Step 1: If a rule is broken, a staff member will first verbally tell the student to stop the inappropriate behavior.
Step 2: If the inappropriate behavior persists, a staff member will remove the student from the activity or situation and call for the camp director to address the situation with the student.
Step 3:If the inappropriate behavior persists, the parents will be called and notified of the situation and asked to speak with their child over the phone.
Step 4: If the inappropriate behavior continues, parents will be called to remove the child from camp immediately. The camp director may allow the child to return to camp the following day, or the child may be removed for the remainder of the session.

The camp director reserves the right to jump to Step 4 and remove a camper if it is deemed to be in the best interest of the Forefront Arts Drama Camp program or for safety of other staff and campers. Immediate dismissal of a camper may result from severe infractions. For example, physical violence of any kind towards another camper or camp staff is grounds for immediate dismissal. No refunds or credits will be given for campers removed for disciplinary reasons.

I understand that every family must agree to the Code of Conduct and Behavior Expectations to attend a Forefront Arts camp, and failure by the student to comply with the Code of Conduct may result in dismissal without refund. I understand that physical violence of any kind towards a fellow camper or staff member is grounds for immediate dismissal.
