Forefront Arts provides classes and training at the exclusive risk of the participants. It is strongly recommended that students are covered by their own family insurance policy. Forefront Arts is not responsible for any damage or loss to personal property or injury suffered during any associated event or activity. Your response below indicates full acceptance and understanding of this liability waiver and all Forefront Arts policies.

I give permission to Forefront Arts to authorize any medical attention needed to my child/student in the event of a medical emergency.

I understand that I am financially responsible for any damages caused by my child to the host location, Forefront Arts property, or the belongings of another student.

Restroom Independence Policy for 3-4 year olds

Camper Restroom Independence Policy *

All campers ages 3-5 will be taken to the restroom every hour and encouraged to use the facilities. Outside of those times, a camper can always ask the teacher to be excused to the restroom.

All campers must be fully potty-trained and independent in the restroom to attend a Forefront Arts camp

Camp staff are not allowed to go into bathrooms with campers or assist with clothing or bathroom needs which is why campers need to be able to handle those needs themselves.

We understand that a bathroom accident can happen in young children. In case of an accident parents will be called to come clean & change the child as our staff is not allowed to do so.

If 2 bathroom accidents take place during the same camp week parents will have the option to remove camper from the camp session or for the child to wear pull-ups for the rest of their camp week. (No refunds or credits will be given for campers not participating due to lack of potty-training)

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 or other health concerns

Health Waiver *

Students and Immediate Family Members are fully aware of the risks and hazards with respect to COVID-19, or any other viruses, inherent in their participation in Forefront Arts programs during the 2021 summer camp season.

Students and Immediate Family Members freely and voluntarily agree to assume the risk with respect to COVID-19, or any other viruses, including the risk of death, bodily injury or property damage, regardless of severity, that they may sustain as a result of their participation in Forefront Arts 2021 summer camp activities.

Any students found to be exhibiting a temperature above 100.4 or presenting Covid-19 symptoms will be immediately sent home until they are able to return, following current state & CDC guidelines.

No refunds or credits will be given for students sent home for exhibiting Covid-19 symptoms but every attempt will be made to offer a virtual theatre camp experience as an alternative until they are able to participate in-person again.

2022 Health & Conduct Expectations

HEALTH & SAFETY PROTOCOLS: (please check understanding & agreement to each item) *

Campers will be dropped off and picked up outside the facility.

Campers (and staff) may wear masks if they choose to, but are not required to wear masks.

Please do not attend camp if you are not feeling well. Anyone who becomes ill while at camp will be sent home.

Camp groups will be kept smaller than normal to allow for physical distancing indoors and outdoors.

Campers will bring all of their own food & water from home

Campers will do hand washing/hand sanitizing throughout the day.

Our staff will sanitize commonly touched surfaces throughout the day.

We will utilize spreading out with outside play as much as possible for our big active games!

All camp activities have been tweaked to ensure kids can have a great time and be safe!

CAMPER CONDUCT EXPECTATIONS: (please check understanding & agreement to each item) *

1. Respect the teacher: campers will follow the directions of the Forefront Arts staff. Campers will participate in all scheduled activities and maintain a positive attitude.

2. Respect each other: campers will treat everyone with respect: bullying, stealing, teasing, cut-downs and profanity will not be tolerated. No touching other campers or camp staff is allowed. Campers will respect the social-distancing guidelines and make adjustments if reminded they need to spread out.

3. Respect our space: campers will not purposely deface the host locations, or purposefully harm or destroy any property, furniture, props, or costume pieces.

PROCEDURES FOR DEALING WITH INAPPROPRIATE BEHAVIOR: (please check understanding & agreement to each item) *

Our three camp conduct rules will be reviewed with all campers every morning during circle time.

Step 1: If a rule is broken, a staff member will first verbally tell the camper to stop the inappropriate behavior.

Step 2: If the inappropriate behavior persists, a staff member will remove the camper from the activity or situation and call for the camp director to address the situation with the child.

Step 3:If the inappropriate behavior persists, the parents will be called and notified of the situation and asked to speak with their child over the phone.

Step 4: If the inappropriate behavior continues, parents will be called to remove the child from camp immediately. The camp director may allow the child to return to camp the following day, or the child may be removed for the remainder of the session.

The camp director reserves the right to jump to Step 4 and remove a camper if it is deemed to be in the best interest of the Forefront Arts Drama Camp program or for safety of other staff and campers. Immediate dismissal of a camper may result from severe infractions. For example, physical violence of any kind towards another camper or camp staff is grounds for immediate dismissal.

No refunds or credits will be given for campers removed for disciplinary reasons.

We want our drama camps to be a fun and safe experience for EVERYONE involved. Parents, please review these expectations with your camper before your camp week. *

2022 Enrollment, Cancellation & Refunds Policies

2022 Summer Camp Participation/Withdrawal/Refund Policies *

I understand that camp registrations may be cancelled in writing by emailing "Register@ForefrontArts.com" until May 1, 2022 with a full refund given, less a $50 administrative fee.

I understand that no refunds will be given after May 1, 2022 but campers may transfer to another Forefront 2022 in-person camp week for a $25 transfer fee if space is available, up to 7 days before the camp start date. I understand that I am responsible for any cost difference between the original and replacement camp weeks if I choose to transfer my enrollment. I understand that transfer requests submitted 7-14 days before the camp start date will be considered on a case-by-case basis and are not guaranteed.

I understand that no transfers or adjustments of any kind will be possible within 7 days of the camp start date, or for any camper who has come to the camp and then chooses to not participate. I understand that not participating because of the instructor or other campers (including their race, sex, age, nationality, or religion) or because of the role received is NOT a valid reason for withdrawal, and no adjustments can be made.

I understand that failure to comply by the Code of Conduct will result in dismissal without refund. I understand that physical violence of any kind towards a fellow camper or staff member is grounds for immediate dismissal.

I understand that Forefront Arts makes arrangements based on student enrollments and this posted withdrawal/refund policy that I am agreeing to before registering my child applies to ALL campers, including my family.

COVID cancellation policy: I understand that all 2022 camp registrations are non-cancellable and non-refundable after May 1, 2022. I understand that Forefront Arts staff will do everything to accommodate a transfer request if my child cannot participate in-person but I understand that ultimately my child's in-person spot cannot be canceled and/or refunded, regardless of whether or not he attends the camp due to covid precautions or illness.
