PLATINUM CONTRACT REQUET FORM

Platinum Contracts include unlimited skating for a monthly flat rate, with two options for the 50-minute session or 80-minute session package. Platinum skaters may request a maximum of 15 sessions per week, with an unlimited balance for walk-on sessions, and the flexibility to freeze and/or cancel throughout the season should an athlete suffer an injury/illness or relocate (which is not applicable through the standard contracting). Sessions at the Daly Rink may be included in Platinum Contracts.

- I understand that all sessions requested on this form must be approved by the Club Office. In the event a session is full or the skater is not eligible, the Club Office will work with each skater on options.
