MONITORING SESSION REGISTRATION

Due to the recent announcement of the U.S. Figure Skating Championship Series moving to a virtual format, The Skating Club of Boston would like to host a monitoring session for our juvenile, intermediate and novice competitors who would have been participating in the Series competitions starting next week. We hope you can join us for these feedback sessions on Saturday, November 14 starting at 2:00pm. Junior and senior competitors may exhibit, however, they will not receive officials' feedback due to future qualifying opportunities for the U.S. Figure Skating Championships.
 
To provide the safest environment for all skaters, officials and coaches, officials will provide feedback on a worksheet that is handed to a staff member and scanned and emailed to the coach and/or skater. We are encouraging dialogue during the session with the coach, but will be utilizing headsets to ensure social distancing.
 
If you were competing in the Series in multiple disciplines and would like feedback for both, please complete two forms.
 
 
REGISTRATION DEADLINE: MONDAY, NOVEMBER 9, 2020 AT 12:00 PM

SKATER | COACH

*In the event a skater at the novice level is notified by U.S. Figure Skating of an opportunity that conflicts with the monitoring session, skater may choose to exhibit and not receive feedback or withdraw from the monitoring session prior to Tuesday, November 10 at 5:00 p.m.  The skater/parent/guardian will email events@scboston.org to communicate the withdrawal.
Each skater is allowed no more than TWO (2) coaches rinkside during the monitoring session.

CHAPERONE

I understand that only ONE (1) chaperone per skater is allowed to enter the building for the duration of the monitoring session. I agree to abide by all safety protocols in place at the time of the event including but not limited to: wearing a mask correctly at all times, and remaining socially distant (at least 6 ft.) from other members, coaches and Club personnel. I agree to enter the building at the time assigned to my skater's group and not before, and leave immediately following my skater's performance and feedback session. *

CANCELLATION ACKNOWLEGEMENT

The Skating Club of Boston welcomes those competitors previously registered in the 2021 U.S. Figure Skating Championship Series in the juvenile, intermediate and novice levels to participate in the monitoring session FREE of charge
 
Withdrawals from the monitoring session will be subject to a $50.00 cancellation fee unless one of the following exceptions are met:
 
(1) *The skater is in the novice level and is notified by U.S. Figure Skating of an opportunity that conflicts with the monitoring session.  Skaters may choose to exhibit and not receive feedback or withdraw from the monitoring session prior to Tuesday, November 10 at 5:00 p.m.  The skater/parent/guardian will email events@scboston.org to communicate the withdrawal.
 
(2) The skater or member of the skater's immediate household tests positive for COVID-19. Proper documentation will be required.
I acknowledge that a cancellation fee of $50.00 will be charged if the skater withdraws from the monitoring session for any reason excluding a novice opportunity and/or a COVID-19 positive test. If my skater is in the novice level, I agree to withdraw him/her from the event by emailing events@scboston.org prior to 5:00 p.m. on Tuesday, November 10. *

VOLUNTARY WAIVER AND RELEASE OF LIABILITY

Using the measures and guidelines provided by the Commonwealth of Massachusetts, the City of Boston, U.S. Figure Skating, and the U.S. Rinks Association, I understand that The Skating Club of Boston has put in place enhanced health and safety measures to reduce the spread of COVID-19. I also understand that the Club cannot guarantee that I or my child(ren) will not become infected with COVID-19. By signing this agreement and visiting and using the facilities of The Skating Club of Boston, I acknowledge that an inherent risk of exposure to COVID-19 exists in any public place where people are present.
 
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Club, and that such exposure or infection may result in extreme illness, permanent disability, and possibly death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Club may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Club employees, volunteers, and program participants and their families.
 
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Club or participation in Club programming (“Claims”).
On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold
harmless the Club, its employees, agents, and representatives, of and from the Claims, including all
liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I
understand and agree that this release includes any Claims based on the actions, omissions, or
negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection
occurs before, during, or after participation in any Club program.
