Application Instructions for the Heart Failure Academy Program

The process for the application will only begin once all required documents have already been completed and validated by our team.

Eligibility Criteria

Candidates who are eligible to apply to Heart Failure Academy Program:

Pharmacists practicing in the hospital setting preferably with prior experience in managing cardiac patients.

General Application Requirements 

All applicants will need to submit and upload the following documents. Must be in PDF Format and must only be in 1 attachment. Zipped files will not be accepted.

  • Completed online application
  • Passport Copy (clear and colored copy)
  • Residency Visa Copy (Non-UAE Nationals) (clear and colored copy)
  • Emirates ID (for UAE Residents, front & back)
  • 1 Photo (Passport Size)
  • Health Insurance Copy
  • If you live in UAE, you may obtain your police clearance online by using the following link: https://www.moi.gov.ae/en/eservices/Issuance.clearcriminalrecord.certificate.348.aspx. Please select Cleveland Clinic Abu Dhabi as the entity requesting the clearance (the beneficiary)
  • For individuals living abroad, please obtain police clearance from the country of your residence
  • Occupational Health requirements* (see below)

Supplemental Application Requirements

All applicants will need to submit and upload the following documents. Must be in PDF Format and must only be in 1 attachment. Zipped files will not be accepted.

  • Letter of Intent (cover letter)
  • 1 Letters of Recommendation (Written in official letter head and duly signed) (must be in PDF format compressed in 1 file)
  • CV

Occupational Health Requirements*

Once you have submitted your application, if selected to attend as an observer, you will be required to provide a Health Clearance Agreement. This will be sent to you in an email from the CCAD Academic Office.

The Health Clearance Agreement must be completed by yourself and a physician witness attesting to your immunization history and that you have received proper immunizations against Rubella, Rubeola, Varicella, and Hepatitis B and that these records are current. In addition, I show no indications of having Tuberculosis (cough, fever, night sweats, weight loss).

Observers cannot start the program without submission of a completed Health Clearance Agreement Form.

Important Notice

Please read carefully

  • All INCOMPLETE and submitted applications will be automatically deleted from our system. Please take time in filling up the form and uploading the required documents.
  • Trainees attending the training are not employed by CCAD.
  • Trainees in the Heart Failure Academy DO NOT receive any financial support, worker's compensation, housing, insurance coverage, or other compensation from CCAD.
  • Trainees in the Heart Failure Academy must follow all bylaws, procedures, and policies implemented at CCAD.
  • Trainees in the Heart Failure Academy must follow patient confidentiality as outlined in policy “Confidentiality of Patient Health Information” (FIN_POL_110).

Trainees in the heart failure Academy are asked to bring their own white coats and comply with professional dress code which is business attire.
