Akilah Aussies Puppy Questionnaire

If you are interested in purchasing one of our pups/dogs we would like you to fill out our questionnaire. As we at Akilah Australian Shepherds believe our pups deserve quality, loving, forever homes. We want to make sure that our pup fits your home and family. If there is more than one buyer we would like each one to fill out the questionnaire. 3) What color or sex do you prefer if any?

4) Have you ever owned an Aussie before? If so what traits do like best about them?

5) Have you ever raised a puppy? If so when? And what happened to it?

6) What amount of time and energy will you be able to spend per day with your pup?

7) What type of environment will you provide for your Aussie?

8) Will the pup be allowed in the house?

9) Where will your pup stay when you are not at home? Where will it stay if you are on an extended trip?

11) How will you provide proper socialization for your pup?

12) Do you have financial capability for proper vet and nutritional care etc.?

13) Will you provide us with feedback on your pups temperament, health and maturation, as well as it’s accomplishments?

14) Will you supply us pictures at 6 months, 1 yr. & 2 yrs?

15) Will you copy us on all Cerf, OFA and other health related clearances?

16) Please list any dog related references. (Veterinarian, trainer, kennel owner etc. ).

20) All of our pups are sold on contract, including but not limited to; mandatory spay/neuter for pets, breeding rights for show dogs and if you can no longer keep our dog for any reason, that it will be returned to us at no cost papers intact, and may not be re-homed, sold or given to anyone else, without express written approval from us. Will you agree to this?

21) If we do not have the pup for you may we refer you to another reputable breeder?

Please Note:
It is important to remember that when looking for that perfect puppy you may need to wait for that litter to be born. A reputable and careful breeder does not have puppies available at all times and a good puppy is well worth the wait. If you are willing to settle for just any puppy, that is just what you will get, any puppy!
