Pre existing medical conditions, activity restrictions, medications.  Do any of the students listed on the registration have any pre existing medical, physical or other condition which there may be reason to restrict safe participation in these skating activities or other strenuos play?

I understand that it is is highly recommended to wear a helmet and safety gears (compulsory for under 18) while attending Skating classes. My submission further acknowledges that I realize that protective gear is designed to reduce the risk of injury, but will not prevent all injuries. In addition I freely assume all risks and release Sportsline, its employees from any liabilities and claims that may arise from my child’s attendance at these classes.
