Optional:  Creating a username and password
You do not have to create a username and password to complete this survey.  However, if you want to stop while you are completing this survey and then return later to finish it, or review your answers after you've submitted them, you will need to set up a username and password.  Also, if there are other people in your family who might complete this survey using the same computer, a username and password will help ensure that their answers are included in the results.

You can create a username that does not reveal your identity if you wish to remain anonymous. 

The form below asks for an email address.  You do not have to provide your email address.  To remain anonymous please enter the fictitious email address any@any.any in the box below.  You only have to provide your actual email address if you want the computer to be able to email you your username and password if you forget them.   If you do provide your actual email address it will remain confidential, will not be shared with anyone, and you will not be contacted by anyone.  I will only contact you if you ask to be contacted and provide your email address at the end of the survey.

Once you have logged in, you will be taken to the beginning of the survey.  To stop at any time, just close your browser.  Your answers will be saved. If you are returning to a survey you started earlier, hit "next" until you get to the page where you left off.
